Cooperative Health to Open New Columbia Dental Office January 11th
Columbia, SC– Eau Claire Cooperative Health Center (dba Cooperative Health) is pleased to announce the expansion of dental services in Columbia. On Monday, January 11, 2021, Cooperative Health will open Cooperative Health Dental at its James E. Clyburn campus, located at 4605 Monticello Road, Building C.
A 2,000 square foot state-of-the-art practice, Cooperative Health Dental will offer oral health services (routine exams, teeth cleanings, fillings, extractions, crowns and bridges, periodontal gum therapy, and sealants) to persons of all ages Monday through Friday, 8:30 a.m. to 5:00 p.m. Insured, uninsured, and/or underinsured patients are all welcome. A sliding fee scale (the Good Samaritan Plan) is available for all patients. All forms of insurance are accepted including Tricare, Medicaid, and Medicare.
Located in Cooperative Health’s founding zip code of 29203, Cooperative Health Dental addresses the need for comprehensive oral health services and education in the Eau Claire community. Its location is within a multi-disciplinary practice setting at the Clyburn campus which includes primary care, behavioral health, internal medicine, a walk-in clinic, and pharmacy services.

This new dental access point will serve low-income and underserved individuals who traditionally do not seek oral health services due to socioeconomic factors. Cooperative Health Dental’s integration into the Clyburn campus enables us to further deliver comprehensive care that addresses nearly all patient needs,” said Peatra Cruz, Chief Organizational Development Officer.
Cooperative Health Dental’s lead provider is Dr. Kimberly Smith, who has practiced dentistry for 16 years. She received a Doctor of Dental Surgery from Howard University and is a member of the American Dental Association, South Carolina Dental Association, and the Greater Columbia Dental Association.
