Request an Appointment

Please do not use this form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact the nearest Cooperative Health office for medical care or the office you normally visit to reschedule an appointment.

After you submit your appointment request, our scheduler will respond within 48 hours.

Once you have made your initial appointment, we recommend that you complete the new patient forms prior to your arrival. You can do this by visiting our patient portal and completing them online.
