Community Assistance Program

Community assistance is offered to Cooperative Health patients who are underinsured, uninsured, and/or eligible for medical assistance.

The program provides support for individuals applying for medical assistance programs to care for themselves or their family members. Our professional staff will help patients complete the application process and determine eligibility for Cooperative Health assistance programs.

The community assistance program is sponsored by Cooperative Health and the United Way.
