Agricultural Worker Health Program

If you are an agricultural farm worker, or you work in crop or animal production or aquaculture, you and your dependents are eligible to enroll in Cooperative Health’s Agricultural Worker Health Program.

Available Services:

• Adult medical services
• Pediatric medical services
• Behavioral health services
• Dental Services

How do I enroll?

1. Call (803) 894-3736, ext. 1557 to schedule an appointment.
2. Bring your most recent pay stub or a letter from your employer stating that you are working on a farm.
3. Fill out a short application form.
4. Pay a small co-pay of $10 for medical services or $20 for dental services.
